POEMS syndrome is a rare paraneoplastic syndrome characterized by polyneuropathy, organomegaly, endocrinopathy, monoclonal gammopathy, and skin changes. The diagnosis of this disease is often challenging due to its rarity and its multisystemic involvement. Isolated adrenal tuberculosis is also a rare disease which is also difficult to diagnose. Therefore it is incredibly challenging to diagnose a patient with POEMS syndrome when he has isolated adrenal tuberculosis. In this report, we describe a 43-year-old man suffering from POEMS syndrome with right adrenal tuberculosis. His symptoms were very complicated and initially we misdiagnosed him with endocrinal disturbances and the adverse effects of antituberculosis drugs. It was not until we accumulated enough evidences of POEMS syndrome that we could make the final diagnosis. To the best of our knowledge, this is the first report of POEMS syndrome with isolated adrenal tuberculosis.
