Abstract: Introduction: Hyperthyroidism corresponds to a dysfunction of the thyroid gland, which secretes hormones in excessive quantities on the one hand and on the other hand clinical and/or biological manifestations of thyrotoxicosis. The objective of our study was to evaluate, through a retrospective study, the anatomical-clinical aspects and the surgical management of hyperthyroidism in the general surgery department of the Ignace Deen national hospital. Material and method: This was a retrospective study of descriptive type with a duration of 5 years from January 1, 2013 to December 31, 2017. This study included all the records of patients admitted to the general surgery department of the Ignace Deen hospital with the diagnosis of hyperthyroidism with biological confirmation during the study period. The diagnosis of hyperthyroidism was evoked in front of clinical signs of thyrotoxicosis and confirmed by the collapse of the thyroid stimulating hormone (TSH) below 0.25 mUI/L and elevation of free T4 higher than 22 pmol/L. Results: During this study period, we collected 6210 patient files, among which 133 cases of hyperthyroidism, i.e. 2.14% of the total number of pathologies received in the department. Predominance of women and an average age of 39.73. Among the signs of compression recorded, dyspnea was the most frequent, followed by dysphagia and dysphonia. We noted abnormalities of tracheal deviation and/or compression on cervical radiography in our study. We had mostly heteromultinodullary goiter, followed by diffuse goiter and basedow disease. Subtotal thyroidectomy was the most used surgical procedure with 69.1% followed by loboisthmectomy with 29.3% and finally total thyroidectomy with 1.5%. Conclusion: In our practice, subtotal thyroidectomy gave good results with an acceptable rate of hypothyroidism and no recurrence of hyperthyroidism in the short term. Radioactive iodine therapy remains a challenge in our context.

Penetrating Wound of the Thorax by Zebu Horns: Report of an Observation

Abstract: Introduction: Thoracic wounds by of zebus horns are rare accidents that occur most often in rural areas. They are characterized by their high sepsis. The objective of this work was to present a clinical case relating to a penetrating wound of the thorax by zebu horns. Observation: It was a 50-year-old patient with no particular pathological history who consulted the emergency room for chest pain, a wound in the chest following a trauma to the chest by zebus horns. On admission the vital parameters were within normal limits. There was a wound of about 2cm at the level of the 7th left intercostal space 2cm outside the nipple line, about 4cm deep, with irregular edges with slight subcutaneous emphysema, the right hemithorax was unremarkable; the abdomen was soft and painless, the examination extended to other organs and systems was normal. A chest X-ray showed a left hemo-pneumothorax. He had emergent surgery consisting of an abundant washing of the wound, a thoracic drainage and closure of the wound. The postoperative course was simple. Conclusion: penetrating wounds of the chest are frequent and serious lesions, zebu horns wounds represent a rare clinical entity found in particular conditions. The care is multidisciplinary, and it begins at the place of the accident. Thoracic drainage, the basic gesture of thoracic surgery, stabilizes most of the victims of this accident. The failure of the drainage must lead to a reassessment of the therapeutic attitude in order to rectify the initial diagnosis and prevent sequellar lesions.

Abstract: The aim of this study was to report our experience on the management of volvulus of the sigmoid colon in our service. Sigmoid volvulus is the twisting of the sigmoid colon around its mesocolic axis. It covered 50 files of patients operated on and hospitalized in the surgery department. Were included in our study all patients operated, hospitalized with well-filled medical records. Patients whose records showed abnormalities were not included in the study. The sex ratio of nearly 3.16 reflected a clear male predominance of the condition. The most affected age group was 35 to 44 years old, i.e. 12 cases with 24 percent. The average age was 28.61 years with extremes of 25 years and 95 years. In our series the farmers were the most affected in 22 cases. According to the history, we noted chronic constipation in 86 percent followed by high blood pressure in 11 cases. We also noted diabetes in 5 patients, tuberculosis in one patient and surgical history in 18 cases. On the clinical level, we noted the arrest of matter and gas in 48 patients, abdominal pain in 47 patients and vomiting in 38 patients. During surgery, 2 rounds were noted in 29 patients, 1 round in 17 patients and more than 2 rounds in 4 patients. The postoperative course was simple in 38 patients and complicated by parietal suppuration in 4 patients. Unfortunately we had noted 8 cases of death. The volvulus of the sigmoid colon is a formidable surgical emergency, the management is medico-surgical.
