The Needs and Services of Elderly Women Based on Kinship in Indonesia: A Qualitative Study

Abstract: The improvement of the health care system has the impact of increasing the population of the elderly in Indonesia. Even though Indonesia still has a small percentage of elderly, slowly but surely this population will increase. The population of the elderly in Indonesia was approximately 18.4 million in 2005, and is expect to increase to 32 million in 2020 or 12% of the total population [1, 16]. Within that population, the percentage of elderly women is greater than that for men. and the life expectancy of women is also higher than men. This research study aims to explore the need for services amongst elderly Indonesians especially women that are based on Indonesian ethnic culture. A qualitative study with focus group discussions involved six elderly women and their families. Literature review and observations have been performed in the different ethnic groups. The result has shown that the elderly are not provided with the proper services and respect, as was formerly the case, especially in the eastern region. They face a range of problems ranging from physical and psychological to social, spiritual and economical. Especially amongst women, they have specific needs as most of the elderly women stay at home and tend to spend their time on spiritual activities such as praying rather than physical and social activities because there is no service provision to ensure their safety and satisfaction. This study recommends that women elderly in Indonesia need more services from their families, government and community based on their values and culture. All parties have to aware of this situation and use the results of this study as a basic template for further appropriate intervention and research.

Abstract: Alzheimer prevalence increases with aging and in 2012 it is estimated that Alzheimer worldwide will affect 35.6 million people, this number will double by 2030 and more than triple by 2050. The aim of this study is to evaluate the effect training program for formal caregivers' dealing with Alzheimer diseased patients at Helwan City Cairo Egypt. Design: A quasi-experimental research design was used. Setting: The study was conducted at four available geriatric homes and out-patient psychiatric clinics in Helwan City. Sampling: A purposive sampling was used for selection of forty formal caregivers'. Tool: A self-designed semi-structured interview questionnaire was used, it included demographic information as; age, sex, professional qualification, and experience, formal caregiver's knowledge regarding Alzheimer disease as definition of Alzheimer, causes, risk factor, prevention and control, attitude questionnaire as I feel confident about people with Alzheimer disease, and I feel uncomfortable being around Alzheimer diseased people and related dementia, and formal caregiver's practices as Practice methods of non-therapeutic care to deal with Alzheimer patients, and Configure appropriate environment to Alzheimer patients Results: The pre /posttests among formal caregivers revealed that overall knowledge, attitude and practices improved with highly statistically significant differences at p <0.001 that approved the hypothesis of implementing training program for formal caregivers. Conclusion: The training program significantly brought out improvements in formal caregivers knowledge, attitude and practices regarding Alzheimer diseased patients. Recommendations: The study can be replicated with a larger number of formal caregivers for generalizations of the results, and further studies conduct to improve nurse's knowledge and practices regarding caring for Alzheimer diseased patients.

Abstract: Communication takes a special form in care and cure aspect which is totally different from the social communication or personal communication. Thus, the this study was to assess the knowledge, attitude and practice regarding therapeutic communication among nurses in selected Governmental Hospitals in Oromia, Western Ethiopia. Institution based cross-sectional study design was employed to collect data by structured self-administered questionnaire from 169 nurses working in hospitals in Oromia, Western Ethiopia and analyzed by SPSS version 20. About 83.3% of study participants have good knowledge, 72.2% of study participants have positive attitude and 75.8% of nurses utilize therapeutic communication. Majority of study participants have good knowledge and positive attitude regarding therapeutic communication.

Abstract: Surgical site infection is postoperative wound infection that occurs after surgery. The complications in the acute care facilities with up to 60% of surgical site infection cases estimated to be preventable by using evidence-based practice. Not with standing the advances in the coronary artery bypass graft, the rising prevalence of surgical site infection has become a primary concern in the infection control practices. The current study amid to evaluate the risk factors associated with the surgical site infections among coronary artery bypass graft patients and, to assess nurses compliance to bundle care to prevent surgical site infection. The study followed a quantitative, retrospective design and was carried out in King Abdulaziz University Hospital in Jeddah city. All sample (n= 120) coronary artery by-pass graft patient's record complied between January 2011 to December 2013. The study revealed statistically significant results between the preference of surgical site infection and demographic factor e.g. body mass index, patient risk factors such as diabetes type1 and 2, selected pre-operative factors e.g. Methicilin-resistant staphylococcus aurous, postoperative factors e.g. rewiring, dressing, ventilation and low cardiac output. This study established that the compliance with the bundle was proven to be effective in reducing risk factors to surgical site infection however nurses adherence to surgical site infection bundle care were incomplete and required more enforcement to comply. Based on the finding, the investigator recommended that nurses should be aware of the importance of adhering to surgical site infection bundles. Nurses should not only be educated on the importance of the bundles, but on how effectives apply the bundles at different stages.

Abstract: Childbirth is physiological process that needs no intervention. Cesarean delivery should only conduct when normal delivery is contraindicated to protect mothers and infant’s health. Nowadays, a Cesarean Section has become a culture for escaping pain. Therefore this study aimed to evaluate the effect of healthy instructions on reducing pregnant women’s fear of normal delivery and preferences for cesarean delivery. A quasi-experimental design was used. A purposive sample of 64 pregnant women was selected and divided randomly into two equal groups, 32 in each group. The study was carried out at Antenatal clinics of Obstetrics and Gynecology Center of Mansoura City. Three tools were utilized for data collection; an interviewing questionnaire and Childbirth Attitudes, Childbirth Fear Associated Factors Questionnaires to assess degree of women's fear of normal delivery and preferences for cesarean delivery. Level of fear was assessed pre and post healthy instructions. The study findings revealed that both groups were significantly different regarding the fear of normal delivery after intervention. It was obvious that before intervention more than half of both groups had preference for Cesarean Section while after intervention, vaginal delivery was the first preference of both groups. This study concluded that, healthy instructions by video and lecture educational methods were effective in making a decision on normal delivery, lessening the fear of normal labor, and diminished the incidence of elective cesarean delivery. The present study recommended to apply healthy instructions by video and lecture educational methods for raising their awareness to distinguish among normal delivery and cesarean section on large number of pregnant women for reducing fear from normal delivery and preferences for cesarean delivery.

Abstract: Learning in the clinical environment is an essential part of nursing education programs. Effective clinical teaching is crucial for the learning of nursing students in the clinical settings. It helps in preparing students for the work they will have to do as practicing nurses [1]. The study aimed to identify the undergraduate nursing students’ and clinical instructors’ perceptions of the characteristics of an effective clinical instructor at the Faculty of Nursing- Cairo University. A descriptive cross- sectional design was utilized of the study. This study was conducted at the Faculty of Nursing, Cairo University. A convenience sample of (591) nursing students and (112) clinical instructors who are working at the Faculty of Nursing- Cairo University and are willing to participate in the study. The Nursing Clinical Teacher Effective Inventory (NCTEI) was used for data collection. Results indicated that the categories of teaching ability and interpersonal relationships were the most valued characteristics rated by both undergraduate nursing students and clinical instructors. While, the least valued characteristics rated by both undergraduate nursing students and clinical instructors were the categories of nursing competence, personality traits and evaluation. The study recommended that workshops/seminars should be organized to orient clinical teachers on their roles in clinical setting. Periodic clinical instructors’ evaluation is necessary to incorporate unused behaviors, into their practice.

Abstract: Aim of this study was to investigate the effect of cognitive behavioral treatment program on anxiety level and self-esteem among secondary school students. A Quasi-experimental pre-post non-equivalent group design was used for this study. The study was conducted at El Manial National Language Schools. A convenient sample of thirty secondary school students was selected. The sample was divided into two groups, fifteen students as the study group and fifteen students as control group. Three tools were used to collect the data for the present study; a) Personal Data Sheet; b) Hamilton Rating Scale of Anxiety; and c) Self-esteem Inventory. A constructed cognitive behavioral treatment intervention was developed by the researcher and implemented to the study group in ten sessions that were held twice weekly, and each session ranged from 60 to 90 minutes. The main study findings revealed that, there was a statistically significant difference between study and control groups in the reduction of anxiety level; however, there was no significant change in self-esteem for both groups. The study concluded that, cognitive behavioral treatment program was effective with secondary school students concerning the reduction of the anxiety level. The study recommended that, there is a great need for continuous follow-up of secondary school students who participated in cognitive behavioral treatment program to support and boost their coping strategies with anxiety.

Abstract: This study aimed to determine the effect of early preparation on anxiety level among patients undergoing upper gastrointestinal endoscopy. A quasi-experimental research design was utilized to achieve the aim of the study. The study was conducted in endoscopy unit of Menoufia University hospital, Egypt. A purposive sample of 300 adult patients who undergoing upper gastrointestinal endoscopy was undertaken. The data gathering instruments were Bio sociodemographic and knowledge assessment form, Beck anxiety inventory scale. There was statistically significant improvement in knowledge related to preparation for upper gastrointestinal endoscopy and reduction in anxiety level in study group more than their control (P<0.001). The study concluded that, knowledge preparation for upper gastrointestinal endoscopy has shown a beneficial improvement of knowledge related to the procedure and reduction of anxiety level among patients. The study recommended that knowledge preparation for upper gastrointestinal endoscopy should form an important concern for patients before performing the procedure that lead to reducing anxiety level.

Abstract: Global warming is one of the most critical environmental problems of 21thcentury. It is not only environmental hazard but also social and economic risk and unfortunately some adverse consequences of it are becoming visible these days. Therefore, it is a great importance for educating people and making them aware of this problem. The study was aimed to examine the effect of instruction with visual materials based on health belief model on global warming perception among university students. Quasi experimental design with pre and post-test was utilized. This study was conducted at six faculties from both scientific and literary faculties in Shebin Elkom City at Menoufia University. Systematic random sample consisted of 360 university students including 144 males and 216 females aged between 18 to 22 years old who enrolled in the first academic years of both selected scientific and literary faculties in Shebin Elkom City. The following tools were used 1. Self-administered structured questionnaire which involved socio-demographic data and knowledge of university students about global warming. 2. Perception of students about global warming based on health belief model. The main result of this study showed that there was significant improvement in the knowledge and perception of university students' about global warming after instruction with visual materials compared to before instruction. Additionally, there was statistically significant relation between students' knowledge and perception toward global warming. The current study concluded that, instruction with visual materials was effective in improving knowledge and perception of university students about global warming. This result recommended that, the need for developing and establishing of environmental protection education programs to enhance environmental awareness of all students from elementary school to university level.

Abstract: Back pain is currently growing as a major health problem among school age group children, which can limit or hinder their daily life activities. The aim of the study was to determine the effect of school bag use, postural and behaviors habits on back pain occurrence among school children. Descriptive correlation design was used. This study was conducted at ten governmental preparatory schools from both rural and urban schools in Menouf district, Menoufia Governorate, Egypt. Random sample of 598 preparatory school children including 239 boys and 359 girls aged between 12-15 years of both rural and urban selected schools. Tools: Self-administered structured questionnaire, back pain assessment questionnaire, back pain & body posture evaluation instrument, behavioral habits questionnaire, school bag use questionnaire, and physiological measurements. The main results showed that 74.1% of the studied schoolchildren had suffered from back pain. Female students had significant risk of suffering from back pain by five times than male students. Among school children with back pain, 51.3% had reported moderate pain, the most affected area by pain was shoulder and about one fourth (24.4%) reported that the pain prevented them from performing daily activities. The mean weight of schoolbag carried by all school children was 7.46+ 2. 62 kg and the mean school bag weight as % of body weight was 14.36% ± 4.22. There was significant relationship between back pain and school bag weight, % of body weight, method and duration of carriage. Also, there was a significant relationship between back pain and improper sitting posture for writing, using computer, and sitting in classroom. The study was concluded that back pain among studied school children was high and the pain influences activities of daily life in about one fourth of students. Most of school children carry school bags more than the recommended weights of their body weight. It was associated with sex, parents' history of back pain, duration of sedentary activities, improper posture adopted in performing daily activities, weight of schoolbag, method & duration of carrying schoolbag. Recommendations: The need for developing and implementing of preventative programs in schools about proper postures during daily activities and guidelines for school bag usage.

Abstract: The aim of this study was to investigate the effect of mindfulness based intervention program on anxiety and depressive symptoms among pregnant women. A Quasi-experimental design was used for this study. This study was conducted at antenatal outpatient clinic at El-Manial Educational University Hospital-Cairo University. A purposive sample consisted of 30 pregnant women with high risk for anxiety and depressive symptoms as confirmed by senior obstetrician according to screening sheet for high risk pregnant women. The sample was divided randomly into two groups; study and control groups (15 each). Three tools were used to collect the data for the present study; a) Personal Data Sheet; b) Edinburgh Postnatal Depression Scale; and c) Perceived Stress Scale. The sample was divided into two groups, fifteen pregnant women as the study group and fifteen pregnant women as control group. Aconstructed mindfulness based intervention program was developed by the researchers and implemented to the study group in ten sessions that were held twice weekly, and each session ranged from 60 to 90 minutes. Abstract: Ethical standards offer a context for behavior assessment, and nursing principles which influence nurses’ goals, and approaches with patient care. The aim of this study to evaluate the Impact of Integrated ethics program on nurses practices in health centers at rural areas in Fayoum region, Egypt. A quasi-experimental design with pre and post-test was utilized. Convenience sample were used to collect 70 nurses from three maternal and child health centers in Fayoum region. Two tools were used to collect the data, First: Self-administered interview questionnaire. Second: A structured practice observational check list to assess nurses' practices. The main results, there was an improvement after the nursing intervention program in all aspects. Concerning nurses' knowledge there was 88.6% of them satisfactory pre- intervention while, post program they became 100.0% have satisfactory knowledge. The present study concluded that, implementation of the integrated ethics program had an efficient improving nurses' knowledge, and practice regarding integrated ethics in the rural areas, with highly statically significant differences in all the tested items between pre/post program implementation (P < 0.001). The study recommended to establishment of in- service training programs, and continuous supervision in rural areas to raise nurses awareness regarding Integrated Ethics and motivate them.

Abstract: Background: Health care organization culture can influenced by its ability to manage human resources and has a strong impact on its performance. Aim: the study aimed to assess staff nurses perception toward organizational culture and its relation to their innovative work behavior at critical care units. Setting: The study was conducted in critical care units at Benha University Hospital. Research design: descriptive correlation design was utilized to conduct this study. Sample: Convenience sample consisted of 76 nurses who worked at the previously mentioned setting. Tools: Two tools were used for data collection; (a) organizational culture assessment questionnaire and (b) Innovative work behavior scale. Results: the study showed that, the highest mean score of nurses perception regarding organizational culture was related to organizational learning (2.51±19.15), while the lowest mean score was related to creating change (12.10±3.26). Also, the highest mean score of innovative work behavior were related to idea championing and idea implementation (8.46±1.91 and 12.30±2.76). Conclusion: more than half of staff nurses had positive perception regarding organizational culture, half of staff nurses had high innovative work behavior and there is a significant correlation between staff nurses perception regard organization culture and their innovative work behavior. Recommendations: it recommended that, enhance nurses organizational learning through designated in-service educational programs, reduce staff nurses resistance to change through communication and support innovation as a job requirement.

Abstract: Contraception is important for reducing the rate of unplanned pregnancy in all ages, some abortions, allowing a spacing of families, and improving the health of women. Studies stressed that successfully using contraception to prevent an unplanned pregnancy is a complex undertaking that requires alignment of multiple domains: knowledge about available methods, personal attitudes and beliefs, subjective norms, and a person's ability to obtain and actually use a birth control method. Advance practice nurses in many settings can prescribe and insert contraceptive devices. The present study aimed to determine women’s knowledge level regarding oral contraception pills (OCP) and intrauterine device (IUD) in Riyadh, and assessing the factors associated with this knowledge; either socio-demographic characteristics or gynecological and obstetrical history. Two hundred women participated, the women aged ≥18 years who agreed to contribute to a study concerning contraception. A structured questionnaire was developed and used to compiled respondent socio-demographic, gynecological and obstetric history. It also assessed defendant’s knowledge regarding oral contraceptive pills and intrauterine device. Most of the study sample (97.5%) were educated and only (2.5%) illiterate. About (75%) were employees, (23%) were bachelors. (66%) Are using or at a point used contraceptives. Among those who used contraceptive methods, (61%) used OCP and (8%) used IUD. The primary source of women's knowledge regarding contraceptives was the health care team; doctor, nurse, and pharmacist. Women’s average knowledge score level regarding contraceptives was relatively low; 39.85 of OCP and 33.40 of IUD. Multiple regression analysis showed that age, educational attainment, parity and use of contraceptives influencing knowledge level regarding OCP or IUD. In conclusion, oral contraceptive pills (OCP) are the most commonly used contraceptive/ than the intrauterine device (IUD) so nurses should develop more effective education about contraceptive methods.

Abstract: To explore the new catheter-special pants design and the application for patients with indwelling catheter after transurethral prostate resection, 156 patients after transurethral prostate resection were divided into two groups randomly: experimental group wearing newly designed pants and control group wearing normal pants after transurethral prostate resection; to compare the state of restlessness and the times patients tearing the catheter secondary fixation tape and pulled out personally unplanned. Experimental group wearing newly designed pants showed only one case of catheter pulled out, none of fixation tape was teared, while in the control group, 9 cases of catheter were pulled out personally, 21 cases of tape teared, causing some degrees of urethral injury, delayed the retention time of catheter. There were significant differences between the two groups. The application of newly designed pants shielding the catheter decreases the incidence of catheter pullout after transurethral prostate resection surgery, avoids urethral injury and increased the retention time, improves the functional recovery with no increased cost, which is worth promoting the application in ordinary clinical nursing.

Abstract: Information and communication technology (ICT) continues to rapidly progress and is integrated in practically all health care professional and research fields including nursing. It is essential that nurses and nursing students acquire competences to successfully utilize ICT in their education and practice to benefit patients and gain more satisfaction from their work. This study was conducted to explore the Challenges of Information and Communication Technology Utilization among Undergraduate community Health Nursing Students in Tanta University, Egypt. A descriptive cross sectional design was used for the study where the entire 4th year students who registered with community health nursing departments were recruited (600 students) to participate. A response rate of 84% (504 students) was recorded. A validated structured questionnaire was used for data collection over a period of four weeks. The data collected were organized, tabulated and analysed using the Statistical Package for Social Science (SPSS) version 20. The results indicated that the challenges of utilizing ICT as stated by the students include slow/poor internet speed/access (84.3%) and lack of ICT facilities (80.4%) especially in the rural communities. Other challenges as stated by the respondents are difficulty in accessing the required information (68.7%) and expensive internet subscription (68.5%). The study concluded that the utilization of ICT among the respondents was associated with challenges. A significant challenge is the access to the required information and the lack of ICT facilities. It is therefore recommended that the faculty and the university management ensure adequate provision and maintenance of ICT devices to the students with corresponding accessibility.
