Abstract: Challenges to patient flow within acute hospital settings has been acknowledged internationally as a growing concern that has the potential to impact upon patient safety, satisfaction and organizational budgetary constraints. The development and introduction of a unique Pediatric Acute Unit pilot program has shown promising results which support its continued implementation. Dynamic in its approach, it has achieved a streamlined flow process, reduced overall length of stay, saved bed days (557.81) and reduced cost (1,158,377 SAR over 4 month pilot phase capturing the seasonal surge period of winter months). Using a mixed method of Plan Do Study Act (PDSA), including quantitative data collection, the team mapped the pilot study through 10 defined cycles of testing, evaluation and refinement. The (PAU) must be examined as a concept set against an overarching patient flow methodology which is part of a whole systems approach underpinning quality of care during the patient’s journey.

Abstract: Menopause play a vital role in woman’s life. Recent study emphasized that physical activities are becoming one of the most important alternative treatments to deal with menopausal symptoms. This study aims to determine the association between level of physical activities and menopausal symptoms among women. A cross sectional study design was used in carrying out this study. The study conducted at King Fahad Hospital of University (KFHU) in its Outpatient Department (OPD) from March to May 2018 with a total sample of 208 women who visiting the previous mentioned setting according to the inclusion criteria. Three tools were used to collect data: A structured questionnaire sheet was developed and used by the researcher after reviewing the related literatures to collect socio-demographic data. A standardized tool (Menopausal Rating Scale-MRS) was used to know the prevalence and severity of menopausal symptoms and a standardized tool (Leisure-Time Exercise Questionnaire-LTEQ) was used to assess physical activities intensity. The results of the present study revealed that more than half of the participants had moderate level of the severity of menopausal symptoms (53.4%). This study also shows that the (82%) of them were more likely to do only mild exercise and only two symptoms namely hot flush/sweating from somatic symptoms and depressive mood under psychological symptoms demonstrated statistical significance with p values 0.002 & 0.029 respectively. Also, it is observed in this study that Body Mass Index (BMI) and the menopausal symptoms which comes under the somatic symptoms provides a high statistical significance with p value of 0.001. The study signifies that more than half of menopausal women (53.4%) were in the normal menopausal age and most women with a normal BMI (69.2%). While signifies the prevalence of joint and muscular discomfort was the highest among the four somatic menopausal symptoms with 95.2% of participants, followed by bladder problems was the highest among urogenital subscales with (88.5%). However, the physical and mental exhaustions was the highest among psychological subscales with 84.1% of the participants.

Abstract: Psychological contract is defined as the unwritten or hidden agreement between both the employers and the employees to ensure reciprocity and safe productive work environment. The concept of the relationship between Psychological Contract Breach (PCB) as one of many recognized antecedents of organizational identification has been emphasised for years. Organizational Identification (OI) is defined as the extent to which both the organisation and the employee’s identities are overlapped. Organizational Identification (OI) has many benefits. For instance, it has a robust effect on the employees’ cognitive abilities, attitude, and behavior including their organizational agility and acceptance to the external and internal environmental forces that induce changes rapidly. Organizational agility (OA) requires rapid adaptation of new services, technologies, and personnel in order to react successfully to sudden shifts or changes in the market. Aim: To determine the relationship between Psychological Contract Breach (PCB), Organizational Identification (OI), and Organizational Agility (OA). Design: A descriptive correlational research design is used. Setting: Faculty of Nursing, Damanhur University. Subjects: The study includes all faculty members N=200, (56) academic staff, (85) non-academic (TA and Instructors), and (59) employees. Instrument: A questionnaire of 40 statements on a 5 point-Likert scale to measure (PCB), (OI) and (OA). Results: The highest mean percent score (86.10±12.86) was for Organizational Identification (OI), while the lowest percent score (66.68±12.0) was for overall PCB dimension. Overall PCB has a negative significant correlation with OI where (r=-0.609) and p=<0.001. In relation to OA, it has significant negative correlation with transactional PCB (r=-0.225), relational PCB (r=-0.169), and OI (r=-0.256). Recommendations: 1. To conduct a qualitative study about Psychological Contract Breach in order to generate more information about employees' expectations. 2. To develop a training program for all the Faculty of Nursing staff members, which can enhance their cognitive abilities to be able to respond rapidly to any sudden changes in the market.

Abstract: Explore the application of case management in gestational diabetes mellitus (GDM). 164 patients diagnosed as GDM from February to November 2016 were set up as control group. 196 patients diagnosed as GDM from February to November of 2017 were divided into intervention group. In the control group, gynecology and obstetrics clinic doctors and nurses directly carried out routine health education, diet guidance and exercise advice. The intervention group adopted the case management model of gestational diabetes mellitus, including six major modules of case selection, overall service needs assessment, goal-oriented case management plan, follow-up management, contact and coordination to obtain services and evaluate case success, GDM women for blood glucose management, comparison of case management before and after GDM patients, the difference between the outcome of pregnancy. GDM case management model could effectively control the blood glucose of patients with GDM, reduce the readmission rate due to poor blood glucose control (P < 0.001), and significantly reduce the insulin used rate of patients with GDM (P < 0.019), but there was no significant difference between the two groups of the outcome of pregnancy (P > 0.05). Case management of gestational diabetes mellitus can effectively save medical costs and play an important role in the management of women with GDM.

Abstract: Ureteral stent is a hollow tube with multiple side holes that is used widely by the urologists in many urological procedures. This study was carried out to determine the effect of nursing Staff development regarding ureteral stent management on nurses' knowledge and practice. A quasi-experimental research design was utilized to achieve the aim of the study. A design of one group pre/post test was used. This study was conducted in urological department of university Hospital in Menoufiya. A purposive sample of 70 nurses were selected. These nurses provided care for patients admitted to the urological department in the previously mentioned setting. I- Structured Interviewing Questionnaire: II- and An Observational Checklist. Study findings shows that none of the nurses in the study sample had satisfactory total knowledge and practice at the pre-program implementation. Although few of them had satisfactory knowledge regarding some aspect, the majority had unsatisfactory knowledge regarding ureteral stent management, patient's teaching and stent removal which shows that there is a need of educational programmes in order to improve their knowledge level. The score of total knowledge and practice were significantly increased immediately after the program, and continued to be higher at the follow-up period, this may indicate that, educational programme leads to gain knowledge, and improve practice which might have an impact on improvement of ureteral stent management. The current study shows that none of the nurses in the study sample had satisfactory total knowledge at the pre-program implementation, indicating that the respondents lacked knowledge. However, the score of total knowledge increased immediately after the program, and continued to be higher at the follow-up phase, indicating that the nurses gained knowledge after the teaching program which was implemented. On the other hand, it was observed that there was an association between age, years of experience, and knowledge, and practice of nurses regarding uretral stent management. As well, age, years of experience, and education were found to have an impact on knowledge scores. Nurses should take initiative to improve their knowledge and practices by using online education, virtual learning, booklets, posters, brochures, charts etc.

Abstract: Objective: To investigate the influence of education level on mental condition and medical coping modes in the elderlies, which may provide some evidence for mental care in health management. Methods: Clinical Symptom Checklist 90 (SCL-90) and Medical Coping Modes Questionnaire (MCMQ) were applied to evaluate mental condition of 160 elderlies who were randomly chosen from the outpatients of a hospital in Shenyang. The patients were divided into 2 groups by education levels, highly educated group (n=71) and non-highly educated group (n=89). Scores of the two questionnaires were compared between the 2 groups, and correlation analysis was conducted between symptoms and medical coping modes within either group. Results: Compared with the non-highly educated group, the highly educated group has lower scores on all factors of SCL-90 and higher scores on all factors of MCMQ (P < 0.05). It was showed by the correlation analysis that “confrontation” was positively related to “psychoticism” in non-highly educated group (P < 0.05) but negatively related to “phobic anxiety” in highly educated group (P < 0.05), and “avoidance” was related to no factors of SCL-90 (P > 0.05), and “acceptance/resignation” was positively related to all the 9 factors of SCL-90 (P < 0.05) in non-highly educated group while positively related to 6 factors of SCL-90 (P < 0.05) except for “hostility”, “phobic anxiety” and “psychoticism” (P > 0.05) in highly educated group. Conclusion: Highly education provides elderly people with more positive medical coping modes as well as better mental conditions.

Abstract: Background: According to Saudi Arabia Attention deficit hyperactivity disorder (ADHD) Society, ADHD is one of the most common psychiatric disorder among children and affect both gender with total prevalence of male: 7.4% and female: 4.2% in Jeddah city. The aim was to investigate the effectiveness of an educational program on exchanging elementary school teachers’ knowledge, attitudes and behavioral practices toward ADHD children. Methods: A quasi-experimental, quantitative, (pre/ post one group) research design was used with 57 elementary teachers recruited conveniently from 4 schools located in Jeddah city and affiliated to Ministry of Education, Saudi Arabia. Data were collected by using 3 main tools which are The Knowledge Attention Deficit Disorders (KADDS) scale, The teacher attitude towards inclusion scale (TAIS), and Teacher Intervention Survey which was used to assess teachers’ perceived familiarity with interventions commonly recommended with students with ADHD. The results: Nearly half of the sample were aged between 31 -40 years with total mean score (35.86±7.09) and (80.0%) of them had bachelor’s degree in general education and there is a significant difference between pre knowledge assessment (11.8±5.9) compared with post (18.2±4.9) with highly significant difference t=-6.81 at P.V (0.000) as well the difference between pre and post behavioral practices of studied sample at p= 0.000. Conclusion: the present study suggests that a brief professional educational intervention can be utilized to greatly increase teachers’ ADHD knowledge, providing a cost-effective, practical solution to address the well-evidenced gap in teachers’ training and knowledge about the disorder. Whereas improving attitudes was minor with no significant difference between pre/post assessment. Therefore, it is recommended to incorporate ADHD training programs into teacher-training curricula either in elementary special or general education, with regular reinforcement through in-service training.

Abstract: Objective: The aim of this study was to explore the effectiveness of interventions provided by Enterostomal Therapist (ET) Nurses to visitors of ostomates. Methods: ET Nurses carried out intervention methods of selection, training, organization of visiting group during enterostomy visiting works, this study summarized and analyzed the influence of various intervention methods on the growth of enterostomy visitor numbers and the work of ostomate visits. Results: The intervention program of ostomate visitors adopted by our hospital could effectively promote the growth of ostomate visitor numbers, facilitate the smooth progress of enterostomy visits, lighten the psychological burden of patients and their families in clinical application. Conclusion: The ostomate visits’ intervention program constructed by ET nurses in our hospital was scientific and feasible. Through strict screening, scientific training, reasonable scheduling, continuous exchange and learning and hospital support, we have created an excellent team which provides high-quality visiting service for patients with enterostomy during perioperative period.

Abstract: To investigate the feasibility of early feeding after lumbar spine surgery in middle-aged and elderly patients. Seventy middle-aged and elderly patients with lumbar disc herniation, lumbar spinal stenosis or lumbar spondylolisthesis after treatments of single-segment lumbar discectomy + spinal canal decompression + interbody fusion after total anesthesia with pedicle screw fixation have been selected from January 2018 to May 2019. They were randomly divided into the experimental group and the control group. The experimental group had semi-liquid diet and water at 2 hours after operation. The control group had semi-liquid diet and water at 6 hours after operation; the first exhaust/defecation time, the incidence of nausea/vomiting after eating, and the incidence of postoperative bloating were compared between the two groups. The first exhaust/defecation time of the experimental group was earlier than the control group (P < 0.05), and the incidence of postoperative abdominal distension was lower in the experimental group (P < 0.05). There was no significant difference in the rate of postoperative nausea/vomiting occurrence in the two groups. (P > 0.05). The middle-aged and elderly patients undergoing general anesthesia for lumbar spine surgery should have faster deflation/defecation within 2 hours postoperatively eating, than 6 hours, to reduce the incidence of postoperative abdominal distension without increasing the incidence of nausea/vomiting adverse reactions.

Abstract: Objective We aim to investigate the effect of manual lymphatic drainage in combination with functional exercise in the recovery from modified radical mastectomy. The outcome measurements are the incidence of lymphedema, function of upper limb, pain and life quality after operation. Methods We included 198 patients diagnosed with breast cancer in pathological examination and having received modified radical mastectomy in the First Affiliated Hospital of Jinan University from January 2016 to December 2017. The patients were randomized into experimental and control groups with 99 cases in each group. The control group was given guidance on functional exercise and health education. The control group, based on the treatment of control group was also given manual lymphatic drainage and the patients or their family members were ensured to master the practice of manual lymphatic drainage so that they could perform it after discharge. The incidence of lymphedema, pain, range of motion of shoulder joint and life quality among patients at three, six, twelve months postoperatively were recorded. Results There was no significant difference in the incidence of lymphedema at three months after operation (P<0.05) while there was significant difference in the incidence of lymphedema, range of motion of shoulder joint, pain score and life quality at six and twelve months postoperatively between the two groups (P>0.05). Conclusions Manual lymphatic drainage in combination with functional exercise is effective to reduce the incidence of lymphedema, difficulty in motion of shoulder joint and pain and numbness of affected limb, and improve life quality after breast cancer operation.

Abstract: The health care delivery system has undergone dramatic shifts in care settings in the recent past. More patients are receiving professional home care following discharge from hospitals, skilled-care facilities and rehabilitation centers. Home care has become an integral part of patient recovery especially among the long stay patients. Skilled nursing care delivered in the patient's home may prevent, foresee or limit costly readmissions back to hospitals. There are few other adverse events which are likely to happen at hospital settings like patient fall, developing HAI etc can be totally prevented in a home environment by one to one care which unloads the financial burden on both sides. There have been number of studies undertaken and many literatures are available on hospital readmission rates for reference. Nonetheless not much focus has been given to this particular area i.e. readmission of patients to hospitals while they are being taken care at home settings. Hence this study helps in understanding the readmission rates in home-based care. The study duration was from Jan 2018 – June 2018 to Jan 2019- June 2019. This was a comparative study and the study group was critically ill patients who were admitted during this time at Health care at home (HCAH), India.

Abstract: Background: cooperative jigsaw learning strategy is a kind of cooperative learning method which has been claimed to minimize the competitiveness in the learning environment by encouraging students for cooperative work. Aim of the present study was to evaluate the effect of the utilization cooperative jigsaw learning strategy on maternity nursing students' attitudes and achievements. Research design: A quasi-experimental study was utilized. Sample: A convenient sample of 310 participants recruited from third-year nursing students at the faculty of nursing at Benha University during the course entitled maternal and newborn health nursing in the academic year 2018-2019. Those students were divided into two groups, control group (160) "lecture group" and study group (150) "jigsaw group". Tools of data collection; 1) self-administered questionnaire, 2) Learning's achievement tool (before and after intervention), 3) Likert attitude scale, 4) cooperative jigsaw opinion sheet, in addition to supportive material related to jigsaw strategy. Results: there were statistically significant differences between jigsaw and lecture groups regarding learning achievements immediately after intervention as p-value≤0.05, also there were statistically significant differences between both groups in follow up test with p-value≤0.001. Additionally, the students in the jigsaw group exhibited a more positive attitude regarding the jigsaw strategy than the students in the lecture group. Conclusion: The jigsaw learning strategy is effective in enhancing maternity nursing students' attitudes and achievements during maternal and newborn health nursing course. Recommendations: Apply cooperative jigsaw learning strategy as a teaching method in nursing student curriculums. Further research is still needed to study the effect of jigsaw learning strategy on clinical achievement.
