Abstract: To shorten the time of first eating of the patients after orthopedic surgery by carrying out improvement project, a project team was set up to identify improvement themes and set improvement goals. Before starting the project activities, 102 patients with orthopedic surgery in our department were selected as the control group and the anesthesia of these patients was spinal anesthesia. Another 161 with orthopedic surgery patients were selected as the intervention group. We compared the time between returning to the ward after surgery and the time of first eating after the surgery. After the implementation of the improvement project, the average time of first eating was 92 minutes. The time of intervention group was shorter than that of the control group (P < 0.05), and the rate of intervention group patients who started to drink within 2 hours after surgery was higher than that of the control group patients (P < 0.05); There was no significant difference in postoperative gastrointestinal side effects between the two groups. Patients who returned to the ward at the end of the N class (22:00-08:00) had their first drink after a long time. The data indicate that by carrying out the improvement of nursing projects, the traditional concept of medical staff can be changed, the diet management and nursing process of patients during the operation period can be regulated. Moreover, the time of first-time eating after surgery for patients with orthopedic spinal anesthesia can be shorten, and the safety of early postoperative diet can be improved, which is good for patients to recover quickly after surgery.

Abstract: Background: In the majority of home and hospital based births, postpartum care is not delivered adequately and are highly limited in Ethiopia in terms of coverage and frequency with which the service should be provided. Objective: The aim of the study was to assess the prevalence and predictors of postpartum care uptake among mothers who gave birth in the last six months in Mertule Mariam District, Northwest Ethiopia. Methods and Materials: A Community based descriptive cross sectional study design was carried out through a cluster sampling method with a total sample size of 565 from January 30 to February 8, 2018. Structured interviewer administered pre-tested questionnaire was used to collect data. The collected data was entered into Epi data version 4.2 and then exported into SPSS window version 24. Bivariate and multivariate analysis were undertaken and information was presented by using simple frequency tables, graphs and pie charts. Result: The prevalence of postpartum care uptake was found to be 19% (95% CI: 15.2, 21.9). Fifty one percent of mothers reported that the main reason for not using postpartum care was an absence of any complication. Knowledge about postpartum care (AOR=6.35, 95% CI: 3.41, 11.82), having four or more antenatal care checkups (AOR=8.59, 95% CI: 4.59, 14.58), experiencing complications in the last delivery (AOR=4.89, 95% CI: 2.52, 9.49) and receiving health information after delivery (AOR=3.57, 95% CI: 1.69, 7.53) were found to be statistically associated with postpartum care uptake. Conclusion: This study revealed that postpartum care rates in this community was very low. Hence, there is an urgent need to assess the quality of Postpartum care (PPC) provided and a need for knowledge and ANC raising program focusing on the schedule.

Abstract: A tertiary neonatal intensive care unit (NICU) in China has carried out a study investigating the safety and accuracy of intracavitary electrocardiogram (IC-ECG) guidance in PICC tip placement in preterm infants. From October 2015 to September 2018, IC-ECG-guidance was applied in 327 preterm infants in our NICU. The positioning of the PICC was performed under ECG-guidance and subsequently assessed by chest X-ray. The frequency of correct ECG-guided PICC-placement in one single attempt, premature infants gender, weight, weeks’ gestational age at PICC insertion was recorded. 327preterm infants(207 male and 120 female)were included in the study with a gestation of 31.3 weeks (25.3 to 36.8) and birth weight of 1498 g (600 to 2560). 327 PICCs tip placement was guided with IC-ECG, all concordant with chest X-ray. 313 (95.7%) were correctly positioned, 228 (72.3%) were correctly positioned on the first attempt, 5 were incorrectly identified to have a short line and 9 were malpositioned. The most commonly punctured vein was the basilic vein (n=228), followed by the superficial temporal vein (n = 31), the axillary vein (n = 25). For 3 years IC-ECG method has proved to be a simple, safe, quick method to assess the correct positioning of the PICC in preterm infants. This is a reliable technique, performing real-time manipulation without any complications.

Clinical data from 32 DVT patients were analyzed retrospectively to identify the potential risk factors. Except for one case who was transferred to another hospital for surgical treatment, all 31 cases received conservative therapy via medication. With the Autar DVT scale applied, 10 cases scored below 10; 11 cases 11-14, and 11 cases 15 or above 15. The preliminary risk factors for DVT included advanced age, concurrent high-risk illnesses, lying in bed, obesity, trauma and surgical intervention. Among these 32 cases, the commonly noted concurrent illnesses were hypertension, coronary heart disease, diabetes, atrial fibrillation, infection and cerebral infarction. Twenty-five cases were combined with three or more concurrent illnesses (78.1%). Considering the risk factor scores for DVT and increasing the level of knowledge about DVT among nursing staff are crucial points in improving the quality of nursing practices. Extended nursing is a significant factor in reducing the risk of DVT among those lying in bed for long periods.

Abstract: Background: Endometriosis is recognized as the ‘disease of young girls’ because it seems to be more common in young girls. Early diagnosis is greatly essential to receive appropriate treatment and avoid complications. Aim of the study: to evaluate severe dysmenorrhea for early recognition of endometriosis among adolescent girls. Subject and Methods: Cross-sectional descriptive design was used in this study. The study included 571 adolescent girls from 4 representative elementary schools in Assiut city. A structured Questionnaire was developed by the investigators and included four parts: Part I: demographic data; Part II: menstrual pattern: Part III. Pain history including: a. pain characteristics. b. Menstrual pain associating symptoms: c. Pain level: using of Horizontal Visual Analog Scale (VAS) 0-10 points. Part IV: finding of Trans-abdominal ultrasound (AUS) for those with severe dysmenorrhea. Field work: The study passed through three phases including assessment, implementation, and follow up phases with a period of 7 months. Results: the majority of the girls were between the age of 13 - 14 years. According to VAS, severe dysmenorrhea was reported in 68 girls (15%). Positive ultrasonography findings suggestive of endometriosis were reported among 57.4% of girls who had severe dysmenorrhea. The majority (71.2%) of those girls with severe dysmenorrhea responded to hormonal treatment while 28.2% not responded. There was a positive correlation between number of symptoms associating dysmenorrhea and the pain level. Conclusions and recommendations: Moderate and severe dysmenorrhea are common among adolescent. Those girls who had severe dysmenorrhea and more than five symptoms associating dysmenorrhea might be suspected to have endometriosis. Further large scale study is essential with the use of more informative investigations as MRI and laparoscopy to gain accurate estimate of the prevalence of the disease and its types.
