Pattern of Distant Metastases and Promising Prognostic Factors in Bladder Cancer: A Population-Based Study

Abstract: The aims of this study were to evaluate the incidence and survival rates of distant metastases in newly diagnosed bladder cancer patients, and to investigate the risk and prognostic factors as well. The Surveillance, Epidemiology, and End Results (SEER) database was analyzed for the incidence and survival rates of patients diagnosed with bladder cancer between 2010–2015. Multivariate logistic regression analyses identified risk factors for distant metastases. The survival rates were calculated by Kaplan Meier analysis. Cox regression and Fine and Grey’s model analyses were used to estimate prognostic factors associated with metastatic bladder cancers. Totally, there were 65541 patients with bladder cancer obtained from the SEER database, and 2812 were diagnosed with distant metastases on initial diagnosis (4.3%). Metastatic distribution information was concluded liver, lung, bone, brain and others. Of these, 1029 patients were bone metastases, 988 patients were lung metastases, 687 patients were liver metastases and 97 patients were brain metastases. As a result, younger age, unmarried social status, Black race, non-transitional cell carcinoma subtypes, poor differentiated grade, increased tumor stage and lymph node stage, absence of surgery to primary sites or unknown were all significantly associated with early distant metastases by logistic regression. Multivariate Cox regression analysis showed that older age, female, unmarried social status, non-TCC subtypes, increased number of metastatic sites and absence of surgery to primary sites were significantly associated with reduced overall survival. These findings in metastatic bladder cancer may help to arrange thorough evaluation and then make individualized treatment plan.

Abstract: Background: At present, the spatial studies of liver cancer are too large for observation, or lack systematic spatial epidemics and statistical analyses, so it is difficult to put forward more targeted regional prevention and control measures. This study takes the natural village of Longkou City, Shandong as a small regional research unit, to systematically analyze the spatial structure and spatial heterogeneity of liver cancer in the small region (village as a unit), and provide a scientific basis for the development of regional prevention and control measures for more targeted liver cancer and its related diseases. Methods: Based on the database of the integrated management information system for chronic diseases and death monitoring in Shandong Center for Disease Control and Prevention, the spatial epidemiology and spatial statistics methods were combined in the framework of geographic information system (GIS). Results: The results of spatial distribution showed that there was obvious spatial heterogeneity on liver cancer in Longkou City, and there were many high incidence and high death areas scattered in the distribution. The spatial scanning and space-time scanning results of liver cancer showed that there were relatively fixed high incidence and high death villages in the small area of Longkou City. Conclusion: This study suggests that the liver cancer in Longkou City has a local geographic risk factor, which may have obvious high incidence area, and time factors have a certain influence on the space area of liver cancer. It is necessary to further carry out the geographical weight regression and risk factors analysis, and expand the study model.

Abstract: Objective: To evaluate effect of humanistic care on gynecological tumor patients. Methods: 112 patients diagnosed as gynecological tumor from January 2017 to June 2018 in hospital. The participants were randomly assigned to control group and intervention group. We use new nursing of humanistic care for the patients of intervention group. In control, the patients had traditional nursing services. The data was collected by medical history, Visual Analogue Scale (VAS), Pittsburgh sleep quality index (PSQI) and Newcastle Satisfaction with Nursing Scales (NSNS). Result: Most of participants had cervical cancer [34 (30.4%) & 41 (36.6%)]. Besides, nursing measure of intervention group had better influence than traditional nursing measure of control group (Pain level, 1.21±0.33 vs 3.04±0.44; sleep quality, 13.74±2.34 vs 10.49±2.51). In assessment of patients satisfaction, the assessments of very well were made by 88 (71.4%) people in intervention group. Conclusion: humanistic care can be effectively improved the outcome of treatment on gynecological tumor patients. The nursing measure of humanistic care made not only pain level of patient in treatment process was reduce but also sleep quality of patient was improved. However, the improvement effect was not strong for the outcome of nursing so that the new nursing measure had low effective rate in the treatment process.

Abstract: Objective: Assess the improvement of cluster enteral nutrition treatment for head and neck cancer patients. Methods: 104 participants was invested to join our study from December 2016 to December 2018. They were randomly assigned to intervention group and control group in study beginning. For intervention group, we provide additional cluster enteral nutrition treatment to them. In the process, we use the questionnaires, interview and follow-up to collect the data of them, the questionnaires included Scored Patient-Generated Subjective Global Assessment and Nutritional risk screening. The data shows the changing of nutrition status of participants in the process. Result: Base on albumin and transferrin of participants, the intervention had better improvement than that of control group (1.97±0.24 vs 1.61±0.20, 56.72±2.33 vs 50.17±2.01). In dietary assessment, intervention group had higher score than control group (11.34±1.61 vs 9.04±1.35). But in nutritional risk assessment, the change gap was not significant in the result. In different complications, the most of complication was nausea and vomiting in the participants [16 (30.77%) vs 7 (13.46%)]. Conclusion: the cluster enteral nutrition treatment had strongly influence for nutrition status of head and neck cancer patients. Moreover, the patients of intervention group had better nutrition status after cluster enteral nutrition treatment. Besides, the cluster enteral nutrition treatment provide better restore to head and neck cancer patients.

Abstract: Objective: to evaluate effect of seamless nursing for complications on gastrointestinal cancer patients. Methods: 266 patients diagnosed as gastrointestinal cancer from June 2016 to March 2019. The control group and intervention group were assigned from participants after they agreed to join this study. We collected the information by different questionnaires and hospital record, the questionnaires included Self-Rating Anxiety Scale (SAS), the Self-rating depression scale (SDS) and Self-rating form of satisfaction. Result: The most of gastrointestinal cancer stage in two groups was early gastrointestinal cancer [57 (42.9%) & 55 (41.4%), p=0.023]. In SAS and SDS research, the improvement was not significant (34.22±1.12 vs 46.32±3.26, 32.38±3.01 vs 46.52±5.84), the improvement had statistical significance. In satisfaction research, the satisfaction of intervention group patients was higher (84.2% vs 81.2%). The number of people in bad level is about the similar as the number. Conclusion: The seamless nursing measure had positive influence in treatment outcome. The seamless nursing measure had better influence for the patients of gastrointestinal cancer. However, the result of improvement was not significant for the patients of gastrointestinal cancer. Besides, the seamless nursing had better influence for satisfaction on patients of gastrointestinal cancer.
