Abstract: Different pivotal works allowed the approval of ibrutinib as a CLL treatment, both in the first line and in relapsed/refractory patients, the adverse effects differ from conventional chemotherapy, the discontinuation rate was 10%, but in different real-life studies, showed a higher number of complications and medication suspension rates of 40 to 50%. To know the reality of the use of ibrutinib in CLL in our environment, we designed this work with the objectives of evaluating the safety profile of the drug with the description of the adverse effects, their incidence, and their severity. The clinical efficacy will also be studied with the determination of the response achieved, calculation of overall survival (OS) and progression-free survival. Also as an additional objective, the discontinuation rate and its causes will be obtained. A total of 215 patients in 26 centers throughout the country were retrospectively analyzed. 189 patients (88%) had a global response, and the majority 58% achieved a partial response. The 5-year overall survival was 60%, with no difference between patients with different numbers of previous lines. The progression-free survival of the entire group was 5.06 years. 44.7% of the population, presented at least 1 adverse effect. The most frequent ones were: bleeding, thrombocytopenia, pneumonia and diarrhea. Others presented: anemia, neutropenia, infections, AF, HTA, arthralgia, rash, opportunistic infections. Most adverse events were mild to moderate in grade and generally occurred within the first 6 months. The main cause of treatment suspension was the appearance of adverse effects, 37 patients had to suspend it, 27 due to adverse effects (72.97%). In our work, we found that ibrutinib, as a single agent, has outstanding activity in CLL, with a significant percentage of overall responses, even in patients with several lines of prior treatment. Most of the adverse effects were of a mild to moderate degree. The cardiovascular effects of TKI, HBP, and AF, are in percentages similar to those reported in other studies. If we analyze the percentage of treatment discontinuation, which is 17%, mostly due to adverse effects, this finding is similar to pivotal studies. In conclusion, our real-life study confirms the important activity of ibrutinib in patients with CLL, in the first line and relapses, highlighting the low percentage of treatment discontinuation in our environment. We believe our work reflects the real life and daily care of patients with CLL, under treatment with ibrutinib in our environment.

Abstract: Background: The Samyang Buldalk Bokkeummyeon or widely known as spicy Samyang Ramen is an instant spicy noodle product of South Korea which has started the spicy-eating phenomenon worldwide. Spicy food consumption also causes an effect on the body as they are stimulants that stimulate the circulation and elevate the body temperature as it will induce sweating and hence make the consumer feel cooler. Objective: The main objective of this research is to study the effect of spicy Samyang noodle consumption on Intraocular Pressure (IOP) in healthy young adults. Methods: A total of thirty (n=30) subjects between the ages of 18 to 30 with a mean age of 23.3 ± 2.5 years were recruited for this study. Subjects were required to consume a small cup of 70g of spicy Samyang Ramen. Intraocular Pressure (IOP) was measured using an i-Care TA01 Rebound Tonometer before and after 10 minutes of consumption. Results: The mean of IOP measured was 14.23 ± 2.43mmHg, increased to 15.10 ± 2.70mmHg after the spicy Samyang Ramen consumption. This increase in the IOP was statistically significant (increase by 0.9 mmHg; p<0.05) using paired t test. Conclusion: However, there is an increase in IOP after intake of spicy Samyang noodle, the difference is not considered clinically significant. Even so, its intake must still be taken in moderation especially in patients who have borderline high baseline IOP.

Abstract: Background: With the development of total knee arthroplasty, patients benefit from the occurrence of postoperative complications that affect the efficacy of the procedure. Among them, periprosthetic knee joint infection is a catastrophic complication of total knee arthroplasty, which is more difficult to diagnose and treat in the clinic and has low patient satisfaction. The ability to clearly diagnose and identify the pathogen at an early stage is an important guide to treatment and is the key to successful treatment. Objective: Traditional detection techniques have the disadvantages of low detection rate, complicated operation and time consuming in the detection of pathogens in periprosthetic knee joint infection, which are difficult to meet the needs of disease detection. Metagenomic next-generation sequencing technology is used to extract the genetic material of pathogens in samples and combine with high-throughput sequencing technology and bioinformatics analysis for detection, which has the advantages of efficient, accurate and sensitive detection of pathogens. Conclusion: Metagenomic next-generation sequencing technology has high application value in the diagnosis and treatment of infectious diseases, and its application in the field of periprosthetic knee joint infection is becoming more and more widespread, and the application of this technology in the diagnosis and treatment of periprosthetic knee joint infection is reviewed.
